Add a note field when declaring an item damaged

We would like there to be an option to add a note when declaring an item damaged from the patron account. Currently, there is no option to do that (see screenshot).

    I second Amy's requests. These are vital for making Damaged widely usable for our system.

    Declare Damaged button in the patron record is great but we also need to have other areas updated to support it such as:

    Adding notes (when and why it was Declared Damaged)
    New SQL report for Damaged items
    New SQL report for Damaged & Paid - just like the Lost & Paid report
    Damaged Transition to Withdrawn.
    PLEASE look into developing some of these.

    I would also like to be able to add a note when you delare an item lost!

    We would use this feature alot in our consortium. If note could auto-datestamp and ID the creator it would be doubly great and increase usage! Thanks!
